Implementation Form Example

This resource is an implementation form used by the University of Kentucky. It is a great example of forms that can be used or designed to help with the implementation process. The form shows consideration options, how often AT is used, and the action steps based on consideration options. Also, this form lists functional areas with an area to comment on demands that the student is unable to successfully perform without assistance. Some of these areas include existence (activities of daily living), communication, body support, travel and mobility, environmental interaction, education and transition, and sports, fitness, and recreation. Then, there is also a section that defines the functional areas.

I think this is a great resource because it is an excellent example of how to determine needs for assistive technology. I like that the form is divided into those functional areas, and then has a place for comments. Allowing someone to write observations or notes as opposed to just checking off on boxes, allows for more detailed to be used in the consideration part of the implementation process. 

This is a piece of the implementation form. It shows the functional area and the area for comments on the students demands.
